Team Manager Duties
Managers, please review the Managers Duties document and be familiar with its requirements. It is important that you understand your role as a manager within the ECAFHL; we are all aware this can be a huge commitment and if you need any assistance or guidance with what is written in the Managers Duties Document please ask your Division Governor.


Game Change Form
Managers, please fill out this form when a Game Change is required after the season has already been scheduled. This form will also be required for all weekday games even if they are scheduled at the beginning of the season so the ECAFHL Governors can track all weekday games. The manager requesting the game change must initiate this form, once both managers have signed off the form with a new game date, time, location; this form must be submitted to your Division Governor for approval. No games can be played until the Division Governor sends back the Game Change Form approved.


Play-Off Agreement Form
Managers, please fill out the Play-off Agreement Form for any and all play-off series. Your Division Governor will notify you of the format for each round, who will have venue and when each round is to start and to be completed. The Manager with Venue is responsible for initiating this form, once both teams have agreed upon all play-off games date, time, location; this form must be submitted to your Division Governor for approval. No games can be played until the Division Governor sends back the Game Change Form approved.


Overage Player Application

Managers, fill out the Hockey Alberta Overage Player Application when you are requesting to have an overage player play on your team. Please ensure you read and understand both Hockey Alberta’s & ECAFHL’s overage player policies and guidelines. Once the application has been completed you will need to submit it to the President of the ECAFHL for review and approval, after the President of the ECAFHL has reviewed and approved it then the form must be submitted to Hockey Alberta for review and approval. Please note no overage player may play in any hockey games until this form is approved by all parties and the player has been added to your official HCR.


Substitue Goaltender Request
Managers, Hockey Alberta may allow during any scheduled Exhibition, League, Provincial or Tournament Game the use of a goalkeeper from another Hockey Team of equal or lower Division or Category if medical evidence or extenuating circumstances shows that a replacement goalkeeper is required by the Hockey Team concerned. Permission must be obtained from the appropriate Hockey Alberta Representative & ECAFHL Representative prior to any goalie playing a game. Please note this form is typically used in rush situations so phoning the required personal is highly recommended.


Hockey Alberta Injury Report
Managers, fill out the Hockey Alberta Player Injury report any time a player requires medical assistance, these forms are to be kept in confidence and to be submitted to Hockey Alberta as required. Should a player sustain a long term injury this form will be a mandatory requirement for both Hockey Alberta and Hockey Canada insurance companies. If you have any questions about this form please contact your LMHA representative for further assistance.


Freedom of Information Form
Managers, we ask that you have every player’s parent or guardian fill out the FOIP Form and submit it back to the ECAFHL Executive with your initial HCR Roster on or before November 1. Failure to do so may result in a fine for your hockey team.
